Brachycephalic (flat-faced) breeds like French Bulldogs, Boston Terriers, and Pugs face the highest risk of difficult birth (dystocia) and are the most likely to need a C-section, because their puppies' heads are proportionally large relative to the mother's pelvis. Toy breeds carrying very small litters and giant breeds carrying very large litters also face elevated, but different, risks. No breed is entirely risk-free, and any female dog being bred should have a pre-breeding veterinary exam regardless of breed.
Why breed matters so much in canine pregnancy
Dog breeds vary enormously in body shape, skull structure, and litter size, and all three factors directly affect how labor is likely to go. Research from the Royal Veterinary College's VetCompass program, which analyzed veterinary records from more than 1,000 UK clinics, found that about 3.7% of female dogs overall experience dystocia (difficult or obstructed labor), but that risk is far from evenly distributed across breeds. Compared to mixed-breed dogs, the same research found Boston Terriers were about 12.9 times more likely to experience a difficult birth, Pugs about 11.3 times more likely, and Chihuahuas about 10.4 times more likely. Nearly half of all problem births in the study required a C-section, and the consequences of an unmanaged dystocia can be severe: the same research found that roughly 25% of puppies and around 1.7% of mothers did not survive difficult births that weren't caught and treated in time.
None of this means these breeds shouldn't be bred or that pregnancy is inevitably dangerous, it means owners of higher-risk breeds benefit from knowing what to watch for and planning ahead with their veterinarian, rather than assuming a natural, unassisted delivery is the default outcome.
Brachycephalic (flat-faced) breeds: the highest-risk group
Brachycephalic breeds, French Bulldogs, English Bulldogs, Boston Terriers, and Pugs, consistently top the list for both dystocia and C-section rates. The reason is anatomical: these breeds have been selectively bred for a wide, rounded skull, but their pelvic canal hasn't widened to match. A puppy's head is often simply too large to pass safely through the mother's birth canal, regardless of how strong or well-timed her contractions are.
For French Bulldogs specifically, published estimates of C-section rates vary depending on the study population, ranging from roughly 40% in some clinical research to over 80% in others, but every study agrees it's the highest of any commonly kept breed. Many experienced breeders plan an elective C-section in advance rather than risk a prolonged, unsuccessful labor. Because brachycephalic breeds also carry higher anesthesia risk due to their airway structure, any surgery, planned or emergency, should be handled by a veterinary team experienced with the breed.
Watch for: active straining for more than 30 minutes without a puppy appearing, more than 2-4 hours between puppies with the mother showing distress, or green or black discharge appearing before a puppy is delivered (a possible sign of placental separation and fetal distress). Any of these warrants an immediate call to your vet or emergency clinic.
Toy and small breeds: the single-puppy problem
It seems counterintuitive, but very small litters can be riskier than large ones in toy breeds like Chihuahuas, Pomeranians, and Yorkshire Terriers. When a litter contains only one or two puppies, each fetus tends to grow larger than it would in a bigger litter, sometimes large enough to create a size mismatch with the mother's pelvis, a condition sometimes referred to informally as "single puppy syndrome." Toy breeds are also more prone to hypoglycemia (low blood sugar) during labor, which can cause weak or stalled contractions.
Watch for: visible exhaustion or weakness partway through labor, contractions that seem to stop entirely, or any signs of disorientation or wobbliness in the mother, which can indicate low blood sugar rather than labor progress.
Giant breeds: large litters, different risks
Great Danes, Mastiffs, Newfoundlands, and other giant breeds generally have a proportionally roomier pelvis and often deliver without assistance. Their risk profile is different: because litters can be large, the uterus can become fatigued partway through a long labor, a condition called uterine inertia, where contractions weaken or stop before all puppies are delivered. Because giant-breed litters take longer to deliver in full, it's easy to mistake a resting phase for a problem, or vice versa, so knowing your individual dog's normal pattern matters.
Watch for: a labor pause longer than 2-4 hours when you know or suspect more puppies remain, or visible straining that produces no progress after 20-30 minutes.
Working and herding breeds
Breeds like Border Collies, Australian Shepherds, and Rottweilers don't carry the same elevated structural risk as brachycephalic or toy breeds, but their high activity levels mean owners should pay close attention to exercise moderation during late pregnancy and the postpartum period. When any breed needs emergency help
Regardless of breed, contact your veterinarian or an emergency animal hospital immediately if you notice any of the following during labor: active straining for more than 30 minutes without a puppy being delivered, more than 2-4 hours between puppies when you believe more remain, a mother who seems weak, disoriented, or collapses, or discharge that is green, black, or foul-smelling before the first puppy arrives. These are not "wait and see" situations, dystocia can become life-threatening for both mother and puppies within a matter of hours.
